Certification Mark Application Forms
Dear Certification Mark Applicant:
Below is an application for the NLGI Certification Mark, plus related forms, for you to fill out and submit. The various forms need to be completed and returned to NLGI for approval and processing. Upon our receipt of these completed forms, our Certification Mark Review Committee will begin examining your application and qualifications. If this is an initial or new Mark request, you would also need to send, along with the forms, a check for the Application Fee. Non-NLGI members pay the Application Fee annually, plus the yearly renewable User Fee for each product.
Please note that you will need to return (1) the application, Form A; (2) the qualifications form, Form Q; and (3) two copies of the Mark user agreement, Form C -- all with proper signatures. These instructions are also detailed under "Guidelines," and other information is included in this packet. Following the review of your application and receipt of the User Fee, you will receive camera-ready artwork of the Mark.
The forms are fairly simple to complete. However, we might mention that some applicants have been uncertain about what to list in the "category" and "performance" columns. Under the Category Group would be either "G" (wheel bearing), "L" (chassis), or "G & L." The Performance Classification would be the actual type of Mark for your product, i.e. GC, LB, or GC-LB.
